JOB SUMMARY Integrate case management functions into the patient care, discharge, and home planning processes by utilizing available resources to enhance quality and cost-effective outcomes. Execute case management activities by coordinating and facilitating transfers among different levels of care, both within the institution and from facility to facility. Apply the American Nurses Association Nursing Administration Scope and Standards of Practice, participate in planning activities to promote Magnet standards in the Nursing Services Department and enforce Joint Commission International (JCI) requirements. Support the mission and vision of the American University of Beirut Medical Center (AUBMC) and that of the Nursing Services Department. ESSENTIAL FUNCTIONS / TASK GROUPS • Provision of Case Management Services: * Greet, receive, and create a positive experience for every patient/family by demonstrating a professional, caring, empathetic and friendly approach following the service excellence standards and guidelines. * Identify eligible patients for case management based on inclusion criteria, including referrals, case complexity, admission frequency, length of stay, and cost considerations. * Act as a patient advocate by coordinating with physicians to develop and follow-up on care and treatment plans, ensuring patient safety, cost-effectiveness, and desired patient outcomes. * Investigate, report, and justify extended critical care patient stays, managing variances and delays to maintain optimal patient care standards. * Promote efficient utilization of clinical resources, mobilizing resources as needed to achieve goals and desired clinical outcomes within established timeframes. * Ensure timely completion of patient lab tests, procedures, and imaging, adhering to established timeframes. * Coordinate and facilitate patient transfers within the institution, to external services or from facility to facility by fostering interdisciplinary communication. * Conduct and promote patient care conferences and multidisciplinary approach to ensure appropriate resource utilization, quality care, and cost-effective outcomes. * Participate in research studies and performance improvement projects to contribute to the advancement of healthcare practices. * Determine required resources based on diagnosis, severity, service intensity, and other relevant criteria, using national and regional length of stay standards and community norms to ensure cost-effective outcomes. * Negotiate funding and insurance issues with patients, families, and healthcare providers to enhance cost effective utilization of services while maintaining quality outcomes. * Perform other related duties as assigned by the Nurse Leader for Clinical Affairs or Director of Nursing. COMPETENCIES Client Focus. Plan Of Care, Communication Skills, Problem Solving, Planning & Organizing, Quality Management, Patient Assessment. KNOWLEDGE / KNOW-HOW Knowledge Knowledge of case management Knowledge in current clinical diagnostic procedures and treatments and skills in applying this knowledge in a review. Knowledge of international nursing services is a plus.   Education: Minimum Education: Bachelor's Degree in Nursing. Preferred Education: Master's Degree in Nursing or healthcare related field. Certifications Commission for Case Management Certification (CCMC) is a plus. Experience: Minimum Experience: 5 - 7 years of experience as a Registered Nurse at AUBMC or 7 - 9 years of experience in a similar healthcare setting. Languages: Minimum Languages: English and Arabic (EEE >/= 32) Preferred Languages: French is an asset.
