Position: Nurse Case Manager Location: Oakbrook, IL Schedule: Monday to Friday, 8-hour shift Type: Full-Time Pay: $34.00 - $36.00 per hour About the Role: We are seeking a compassionate and experienced Nurse Case Manager to join our team. In this role, you will serve as a key advocate and care coordinator, developing and managing comprehensive care plans to promote the health and well-being of our patients working on-site in a clinic base setting. Key Responsibilities: Develop, implement, and revise individualized patient care plans in collaboration with patients, families, physicians, and healthcare teams. Act as the primary point of contact and advocate for patients and their families, facilitating access to medical providers and community resources. Provide essential health education to patients and families to improve understanding and management of chronic conditions. Conduct thorough assessments to identify unmet health and social needs. Ensure medication adherence and compliance with follow-up appointments through ongoing support and communication. Maintain timely and accurate documentation and communicate effectively with all members of the care team regarding patient status. Qualifications: Required: Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of Illinois. Minimum of 3 years of nursing experience. Minimum of 2 years of experience with geriatric patients. Ability to commute to and work on-site in Oakbrook Terrace, IL, 2+ years of experience in Case Management or Care Coordination. Preferred experience in a Clinic setting. Experience in post-acute or long-term care settings within the last 5 years.
