SKILLS

  • Strong knowledge of anatomy and physiology relevant to client care.
  • Experience in community, homecare, assisted living or nursing home settings is highly desirable.
  • Proficiency in medication administration practices.
  • Ability to supervise staff effectively while fostering a supportive team environment.
  • Experience in complex care such as tracheostomy, ventilation, suctioning, gastrostomy care, epilepsy, and seizure management (desirable)
  • Experience in supporting people with mental health, learning disabilities and other cognitive impairments.
  • Experience in working with babies, children, or adults.
  • Excellent communication skills to engage effectively with clients, families, and healthcare professionals.
  • Strong organisational skills to manage multiple cases efficiently while ensuring high-quality client outcomes.

FA365 Homecare, a progressive homecare provider of complex care packages, is seeking a Nurse Case Manager to join their growing team. This position offers the chance to travel to different locations in the UK as the company expands its services.

WHAT YOU’LL DO:

  • Assess and manage new client cases (adults and children) with complex mental health or learning disability needs.
  • Create personalised care plans based on the Positive Behaviour Support (PBS) framework.
  • Provide face-to-face support to clients and their families.
  • Train and mentor care staff on clinical skills and activities.
  • Collaborate with the Clinical Registered Manager and core team.
  • Oversee existing client packages and work to acquire new ones.
  • Covering out of hours / on-call duties on a rotational basis.
