JOB SUMMARY The Nurse Case Manager supports student health and success by managing a caseload of students with complex or ongoing medical concerns that may affect their academic and campus life. This role combines direct nursing care, individualized health education, and comprehensive case management. Responsibilities include maintaining regular contact with high-need students, triaging walk-in and phone concerns, coordinating referrals to community providers, managing student vaccination requirements, and collaborating with RISD staff, faculty, and administrators to promote student well-being and continuity of care. ESSENTIAL FUNCTIONS/DUTIES: Triage presenting concerns, provide nursing assessment, and refer to a Nurse Practitioner as needed. Manage incoming students' immunization compliance to ensure compliance with state regulations and institutional policies. Monitor, track, and report on student immunization compliance using the electronic medical record. Monitor medical providers’ inbox to ensure timely evaluation of critical results. Receive high-risk referral orders from the medical providers and the director to ensure students receive appropriate follow-up and understanding of clinical care plans. Coordinate care and transportation for students in emergent or urgent situations within the Health Services department. Review on-call reports received from campus partners, ensuring outreach, timely follow-up and continuity of care. Provide health counseling and education on a range of topics, including but not limited to: nutrition, eating concerns, alcohol and substance use, contraception, STI/HIV testing and prevention, and wellness promotion. Provide follow-up education and evaluation after a medical provider visit, including blood pressure monitoring and injection teaching. Support campus wellness initiatives and contribute to outreach efforts addressing emerging health trends within the student population. Follow specific provider standing orders to administer immunizations, order asymptomatic STI testing, perform pregnancy testing, and administer emergency contraception to the student population. Manage partnership with RIDOH to continue participation in the State Supplied Vaccine program, bringing free vaccinations to students on campus. Keep health education materials, brochures, and online resources current and responsive to student needs. Facilitate medical leaves of absence and returns, including documentation review, provider coordination, student interviews, recommendations, and follow-up support. Maintain accurate, timely documentation of student encounters and care plans in the EMR. ADDITIONAL FUNCTIONS: Performs other related duties as assigned. Stay current with best practices in college health and case management by attending workshops, conferences, and trainings as approved. Participate in quality improvement initiatives and departmental projects. JOB QUALIFICATIONS/SPECIFICATIONS KNOWLEDGE/SKILLS/EXPERIENCE REQUIRED: Registered Nurse professional license, current in state of Rhode Island required BLS certification required Minimum five years previous nursing experience required Bachelor’s in Nursing (BSN) preferred Experience in adolescent, young adult and/or college health or ambulatory care setting preferred Experience in crisis response and management preferred Demonstrated ability to communicate effectively, organize complex information, and work collaboratively on a multidisciplinary team. EMR experience required Experience working with diverse populations, including international students, students from ethnically and racially diverse backgrounds, and LGBTQ+ students. Strong attention to detail and the ability to synthesize information from multiple sources to make informed recommendations for student well-being. Familiarity with HIPAA, FERPA, and college health standards of practice.
