We currently support over 1,000 persons in 12 counties in Middle and West Tennessee. We are currently seeking a Licensed Practical Nurse (LPN) to serve as our Nurse Case Manager / Scheduling Coordinator. This role combines clinical nursing oversight with staff scheduling coordination, ensuring the people we support receive safe, consistent, and high-quality care. This position works closely with the Director of Nursing to manage nursing schedules, coordinate coverage, and support healthcare services for the people we support receiving care. Key Responsibilities Manage and maintain the monthly nursing schedule using HomeBase and adjust as needed based on call-outs, PTO, or staffing needs Coordinate staffing with the Director of Nursing to maintain staffing levels Communicate schedule updates and shift coverage with nursing staff Provide nursing care including medication administration, monitoring effectiveness, and reporting adverse reactions, as needed. Assist with implementing Individualized Health Care Plans and Medical Crisis Plans Assist with training, mentoring, and shadowing new staff Maintain accurate documentation, medical inventory, and compliance with care standards.
