We are currently seeking an experienced Registered Nurse, Nurse Consultant for Toronto (downtown) and Mississauga, ON. The successful candidate will provide services and have the willingness to travel within the area listed above.

KEY QUALIFICATIONS:

  • Registered Nurse in good standing with the College of Nurses of Ontario
  • Demonstrated knowledge of best rehabilitation practices and case management principles
  • Clinical experience with spinal cord injury and traumatic brain injury considered an asset
  • Superior interpersonal relations, communication, problem-solving, and time management skills
  • Demonstrated ability to work collaboratively as a member of a multidisciplinary team
  • Experience developing OCF-18 treatment and assessments plans
  • Self -motivated with demonstrated ability to manage a busy and varied caseload
  • Must have valid Drivers Licence, auto insurance and a reliable vehicle
  • Completion of a satisfactory Police Vulnerable Sector Check
Responsibilities
  • Case Management, guided by the company’s Client Pathways ™ service model, which encompasses professional resources management, goal setting, team management, case budgeting and third party funding approvals
  • Complete hospital discharge plans
  • Setting rehabilitation goals in consultation with clients, their families, case teams and other treating health professionals
  • Preparing and submitting OCF-18 forms for SABS-covered injuries
  • Facilitating medical evaluations and home modifications
