JOB OVERVIEW

Are you an experienced and dynamic nurse looking for your next challenge? Do you have a broad clinical background and a passion for ensuring patients receive the right care, at the right time, in the right place?
The Single Point of Access (SPA) team is seeking a Band 6 Nurse Coordinator to join our dedicated service. This is an exciting opportunity to be at the forefront of patient care coordination across multiple specialities and services.
This role is Par time 28 hours, 8 months Fixed Term (maternity cover).

Responsibilities

MAIN DUTIES OF THE JOB

  • Significant clinical experience across a range of specialities
  • Strong decision-making and problem-solving skills
  • Excellent communication and teamwork abilities
  • A patient-centred approach with a commitment to delivering high-quality care
  • The ability to work well in a fast-paced and ever-changing environment
  • Triage and coordinate referrals to appropriate services
  • Liaise with health and social care professionals to ensure seamless transitions of care
  • Provide clinical advice and support to colleagues and referrers
  • Contribute to service development and continuous improvement initiatives
