Description Gateway Rehab Center (GRC) has an opening for an LPN or RN for our extended care program in Waynesburg, PA. In this role, you will provide assessment, nursing diagnosis/treatment planning, and direct nursing care to GRC patients. This position receives supervision and instruction from the Executive Regional Director. Responsibilities Completes new patient admissions. Provides direct patient care including medication administration. Documents patient assessments and nursing care rendered. Intervenes in crises and other emergency situations. Completion of audits as required. Interacts with the physician through coordination and the provision of assistance via in person, telephone, med wand or other platforms. Complete medical triage, document and report findings to medical staff. Training of PCT’s/Tech 4s to assist with self-administration. Sign off patients to self-administer medications. Collaborate with pharmacy on copays/refills and relay information to patients. Coordinates with all GRC departments as appropriate. Attends GRC mandatory in-services and trainings. Knowledge, Skills, and Abilities Excellent oral and written communication skills Strong computer skills. Requirements Pennsylvania or Compact RN/LPN unrestricted license. One year nursing experience, addiction medicine experience preferred. Active CPR certified required. Work Conditions Working conditions include potential high-risk exposure to blood borne pathogens and low-risk exposure to respiratory infectious agents. Moderate physical demands. Significant mental demands. This position requires the operation of various office and safety equipment. This position requires the competent use of various healthcare equipment and measurement devices. This position requires mandatory staffing coverage when necessary to provide a safe treatment environment for GRC residents.
