Health Yeah! is an equity-centered mobile health program that provides primary and behavioral healthcare to Albuquerque public schools through mobile units and a brick-and-mortar clinic. The program is designed to address healthcare disparities, increase access to care for school-aged children and their families, and promote wellness in underserved communities.

CARE COORDINATION & HEALTH EDUCATION

  • Conduct comprehensive health assessments, including physical, mental, and social health evaluations, to determine healthcare needs.
  • Develop and oversee care plans in collaboration with the Medical Director, ensuring integration of medical, behavioral health, and social services.
  • Provide health coaching and education on disease prevention, nutrition, reproductive health, and mental wellness tailored to school-aged populations.
  • Act as a liaison between patients, school staff, behavioral health professionals, CHWs, and APPs to facilitate seamless, integrated care.
  • Assist students and families in navigating insurance enrollment, referrals to specialty care, and accessing community resources.

QUALIFICATIONS AND EDUCATION REQUIREMENTS

  • Must possess an active, unrestricted RN license in the state of California.
  • Bachelor of Science in Nursing (BSN) preferred; equivalent experience will be considered.
  • Minimum of two years of nursing experience, preferably in community health, school health, public health, or pediatrics.
  • Experience supervising medical assistants or other clinical staff is preferred.
  • Experience as a Nurse Manager
  • Active Basic Life Support (BLS) certification required.
  • Experience with case management, care coordination, or population health is a plus.

PREFERRED SKILLS

  • Strong understanding of pediatric and adolescent healthcare, including immunizations and chronic disease management.
  • Experience working with diverse and underserved communities with a commitment to health equity and social justice.
  • Knowledge of trauma-informed care, behavioral health integration, and harm reduction strategies.
  • Bilingual in Spanish/English preferred but not required.
  • Strong communication and patient engagement skills, with the ability to build trust within school communities.
  • Familiarity with local healthcare resources, Medicaid, and school-based health programs.
  • Proficiency in electronic health records (EHR) and data tracking for quality improvement.
  • Ability to work independently in a mobile setting while effectively collaborating within a multidisciplinary team

Responsibilities

PURPOSE OF THE POSITION

Wellness Equity Alliance seeks a licensed RN in California to provide to oversee clinical operations, care coordination, and quality assurance within the two programs. This position will support services working with youth in our community-based wellness programs. They will work closely with the Medical Director to ensure high-quality, evidence-based care is provided across mobile and fixed clinic sites. As a key member of the multidisciplinary team, which includes behavioral health professionals, community health workers (CHWs), and advanced practice providers (APPs), the RN Manager will facilitate seamless care delivery. Additionally, they will provide direct supervision to medical assistants (MAs), ensuring adherence to clinical best practices and operational efficiency.

Key Responsibilities Clinical Care & Program Oversight

  • Provide direct clinical care, including physical assessments, vaccinations, medication administration, and chronic disease management for school-aged children and their families.
  • Ensure compliance with CDC, state, and organizational protocols for vaccine handling, medication storage, and infection control.
  • Supervise and support medical assistants, ensuring proper training, competency, and adherence to clinical workflows.
  • Maintain accurate and timely documentation of patient encounters, assessments, and treatment plans in the electronic health record (EHR).
  • In collaboration with Operation Manager, coordinate mobile health logistics, including scheduling, clinical supply management, and ensuring a safe, efficient workflow in both mobile and brick-and-mortar settings.
  • Assist with emergency response efforts, including managing adverse reactions and acute medical issues.
