Nurse Navigator (Home Health Palliative Care) Entity: Aga Khan University Hospital Location: Karachi, Pakistan Introduction: The Aga Khan University Hospital in addition to its five Hospitals creates access through its Outreach Services which include Clinical Laboratories, Outpatient Medical Centres, Pharmacy services, Home Health and other services. Outreach Services are designed to provide primary to secondary healthcare services to patients near their homes and are present in several cities across Pakistan. Outpatient Medical Centres offer a range of specialty clinics including Family Medicine, Internal Medicine, Endocrinology, Dermatology, Gastroenterology, Cardiopulmonary, Orthopaedics, Obstetrics and Gynaecology, Paediatrics and Physiotherapy, and cater to children, adults, and elderly patients for general health checks or specific disease consultations. Responsibilities: You will be responsible to: Ensure to provide access to all hospitalized patients to HHS for continuity of care. Identify potential patients, plan their visits and explain home health services Apply advance nursing judgement determine comprehensive care needs Provides and reinforces education to patient, families, and caregiver about diagnosis and post-treatment continuity of care. Performs detailed counselling of patient and family before coordinating plan of care; counselling contains coverage of components of holistic care Supports a smooth transition of patients from hospital care into home care. Maps continuum of care of patient while anticipates future needs Acts as a liaison between the patients, families, and caregiver and the providers to optimize patient outcomes. Facilitates communication among members of multidisciplinary palliative care team to prevent fragmented or delay care that could adversely affect patient outcomes especially in discharge planning. To coordinate care with patient family about details of visiting staff and timings etc. Obtain home health consent and coordinate with HHS team leaders once discharge plan is finalized for patient to be inducted in HHS Coordinate with support services such as PBSD, safety and security, pharmacy etc. Requirements: You should have: BScN degree from a recognized school of nursing and a valid registration from Pakistan Nursing Council at least two years of experience (preferably of Oncology and Medical Surgical Unit) as a Registered Nurse Certification in Basic Life Support, Safe Medication Administration, Intravenous Cannulation Good interpersonal skills, particularly communication skills Sound clinical knowledge and relevant competencies Ability to effectively manage patient counselling and coordinate with multidisciplinary teams. Comprehensive employment reference checks will be conducted.
Responsibilities
