Position: Nurse Navigator, Outpatient Services Entity: Aga Khan University Location: Karachi, Pakistan. Job Role / Responsibilities: You are required to: provide education and resources to facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum work as a personnel who ensure timely access and coordination among different health care teams be responsible for appropriate scheduling and coordination of follow-up care and tracking patients to ensure completion of recommended treatment plan on individualized basis. Eligibility Criteria / Requirements: You should have: BScN Degree from a recognized nursing institute minimum 2 years of experience as a registered nurse/critical care nurse or equivalent experience; experience in Head and Neck would be preferred valid registration from Pakistan Nursing Council certification in Basic Life Support, Safe Medication Administration, Intravenous Cannulation, Advance Life Support (Preferred), Conscious Sedation (Preferred) good interpersonal and communication skills ability to asses, plan and coordinate patient care as per planned management sound clinical knowledge and competencies in specialty specific site persuasive skills to manage patient counselling effectively.
Responsibilities
The Nurse Navigator provides education and resources to facilitate informed decision-making and timely access to care throughout the cancer continuum. They are responsible for coordinating health care teams, scheduling follow-up care, and tracking patients to ensure treatment plan completion.
