Monthly visit Conduct scheduled monthly visit to members. Submit complete notes and documentation within 24 hours. Ensure accurate ADL documentation in care logs. B. Semi Annual Health Status Complete detailed report every six months. Coordinate with member PCP. Ensure compliance and required signatures. C. Plan of Care Develop and update individualized plan with member and caregiver input. Adjust and update as needed. D. Annual MDS Review and update Minimum Data Set. Conduct psycho-social and fall risk assessments. Update Plan of Care E. Monthly MDT Meetings Participate in case review Present Plan of Care goals and coordinate care with all providers What we are looking for: Licensed nurse (LPN and RN) Home care or community-based nursing experience preferred. Strong documentation compliance and time management skills. Reliable transportation. Familiar with patient information systems.
