Description Heartline Staffing is a Strategic Talent Partner with Purple Cow Recruiting. We are seeking a Nurse Practitioner to provide value-based primary care to adult patients in an outpatient clinic setting. This role focuses on longitudinal care, preventive health, and care coordination with the goal of keeping patients healthy and out of the hospital. Nurse Practitioners see patients independently while collaborating with physicians and an interdisciplinary care team. This position allows providers to focus on patient care within the clinic environment, including primary care, care coordination, and population health initiatives. Core Responsibilities include: • Providing high-quality primary care in an outpatient setting • Conducting routine and acute office visits • Completing Annual Wellness Visits and Health Risk Assessments • Coordinating care with specialists, testing facilities, and external agencies • Supporting population health and preventive care initiatives • Educating patients on diagnoses, care plans, and treatments • Assisting with phone triage and patient outreach • Participating in community engagement and promotional activities • Conducting home visits as needed • Collaborating closely with physicians, nurses, care managers, and support staff Compensation: Base Salary: $105,000 – $140,000 (wider range available based on experience and location) Bonus: Up to $8,000 annually Additional compensation may be available for language-critical markets. Requirements Required Qualifications: • Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or Nurse Practitioner Certificate Program graduate • National certification in at least one of the following specialties: – Family Nurse Practitioner – Adult-Gerontology Primary Care Nurse Practitioner – Adult Nurse Practitioner – Gerontological Nurse Practitioner • Active, non-probationary Nurse Practitioner license • Active DEA license • Spanish fluency Preferred Qualifications: • Experience in primary care, internal medicine, geriatrics, or family medicine • Experience with HCC documentation, ICD-10 coding, and Health Risk Assessments • Interest in population health and team-based care • Bilingual proficiency beyond Spanish where applicable Heartline Staffing is a Strategic Talent Partner with Purple Cow Recruiting.
