Description

Your Opportunity:
The Hospitalist service is seeking a Nurse Practitioner (NP) to join their dynamic team at South Health Campus (SHC). SHC is more than just a hospital, it is a campus environment which includes inpatient, outpatient, prevention and public health learning for the community, and wellness services. SHC has four key pillars of service: Collaborative Practice, Wellness, Patient and Family Centered Care, and Innovation. This NP position will provide frontline clinical care to patients with presentations that include a wide variety of acute and chronic conditions in an acute care inpatient setting. The NP will collaborate with the Hospitalist Team, nursing staff and leaders, allied health team members, subspecialists and SHC administration in providing comprehensive, holistic and patient-family centered care. The NP will act as an advocate and will contribute to collaborative inter-professional patient care through advanced clinical practice, leadership, consultations, referrals, education, and research. The NP will perform advanced comprehensive health assessments; order, interpret and perform diagnostic tests; provide input related to semi-complex patients in acute care and clinic areas; communicate assessment findings/diagnoses including outcomes and prognoses; prescribe and distribute pharmaceuticals, medical devices and other therapies; and utilize complex reasoning to safely coordinate and manage care for patients while promoting continuity across the continuum of care. The NP requires strong interpersonal skills and ability to build collaborative and trusting relationships and will demonstrate skills in the areas of organization, critical thinking, conflict resolution, advanced problem solving, interdisciplinary leadership and change management. A passion for caring for patients with medical complexity, and their families, is essential.
