JOB SUMMARY

We are seeking a dedicated, compassionate and experienced Nurse Practitioner to join our team. The ideal candidate will possess a strong clinical background and a commitment to providing high-quality patient care. The Nurse Practitioner (NP) will play a pivotal role in delivering comprehensive, client-centered care to individuals experiencing homelessness and concurrent mental health and addiction challenges. The NP will provide primary and episodic care, mental health and addiction support, and facilitate access to housing and social services.
This role requires expertise in assessing, diagnosing, and managing patient health issues while collaborating with other healthcare professionals to ensure comprehensive care.
This is a permanent part time to full time position working Monday to Friday.

EXPERIENCE

Candidates should possess the following qualifications:

  • Experience in treatment of health conditions for patients with addiction and related health issues.
  • Experience in a community health setting or combination of community health and hospital or public health settings
  • Strong attention to patient and family-centered care and establishing therapeutic relationships is required
  • Demonstrated ability to work in an inter-professional team environment is required
  • Strong communication and excellent interpersonal skills, combined with good analytical, problem solving, organizational and administrative skills
  • Proficient in the use of computers and various software applications is required

EDUCATION

  • Undergraduate degree in nursing from a recognized university (or equivalent combination of education and experience).
  • Registration in the Extended Class with the College of Nurses of Ontario.
  • Member in good standing in the Extended Class with the College of Nurses of Ontario
  • Member in good standing with the Nurse Practitioner’s Association of Ontario (NPAO)
  • Proficiency in the use of computers and various software applications.
  • Participates in CNO Quality Assurance Program.
  • Has and maintains CNO Professional Liability Insurance.
  • Experience with marginalized populations an asset.
  • Completion of a Police Clearance and Vulnerable Sector Check prior to commencing employment

Responsibilities
  • Perform assessment, diagnosis and treatment, health education/counselling and other clinical activities according to the College of Nurses of Ontario standards of practice for Registered Nurses in the Extended Class (NPs) as well as the Medical Directives.
  • Work collaboratively with prescribing physician to provide optimal care
  • Evaluation of treatment, education, counselling and health promotion programs and services as appropriate
  • Provide clinic assessments addressing the physical and emotional needs of clients with multiple comorbid conditions
  • Chart documentation as per college standards
  • Make referrals to support client’s needs and goals
