Description

MOSAIC Community Clinic (MCC) opened in the Burnaby Edmonds neighbourhood in February 2025 and is looking to hire Nurse Practitioners with flexible offerings for the positions.
The clinic operates within the Burnaby Primary Care Network (PCN), a community-based network of healthcare professionals and clinics who plan and deliver the healthcare needs of a community. PCNs are part of BC’s Primary Care Strategy that focuses on implementing team-based care.
The MOSAIC Community Clinic offers team-based care by family physicians, nurse practitioners, social workers, lactation consultants and registered nurses who are also supported by a skilled administrative team, including medical office assistants, a clinic manager, clinic director and medical director.
The MCC provides primary care services including routine check-ups, immunizations, chronic disease management, women’s and children’s health services, specialist referrals and patient education.

The MCC offers culturally safe, trauma-informed health care to diverse priority populations who need it most. These various populations include:

  • newcomers (in Canada for less than three years)
  • persons who identify as First Nations, Inuit and Métis
  • new mothers and their babies
  • individuals with complex mental health conditions
  • individuals experiencing homelessness or precarious housing.

QUALIFICATIONS, EDUCATION AND SKILLS:

Minium Education - Masters Degree - Nurse Practitioner
ALCS; BLCS; CPR; Driver’s License

Responsibilities

RESPONSIBILITIES:

  • Provide high-quality primary care to diverse patient populations.
  • Collaborate with a multidisciplinary team to ensure wrap around care.
  • Engage in trauma and violence informed longitudinal primary care services.
  • Adhere to the principles of a Patient Medical Home, ensuring patient-centered care.
  • Provide outreach services as needed.
  • Coordinate care with specialized health services, hospitals, and other community services.

ROLE AND COMPENSATION:

  • PCN NP Contract (group or individual) –patients are attached to the clinic, not the provider
  • Up to 2.0 FTEs available
  • Flexible full-time or part-time positions (as low as 0.2 FTE)
  • Annual salary is $181,192 (based on 1.0 FTE), part-time positions are prorated based on the above salary
