About the Role The Nurse Practitioner provides comprehensive primary care services to individuals and families across all stages of life. This role focuses on disease prevention, health promotion, and management of acute and chronic conditions within a team-based care model. The ideal candidate is clinically strong, patient-centered, collaborative, and comfortable working in a fast-paced healthcare setting serving diverse and underserved communities. What You’ll Do Key responsibilities include: Perform patient assessments, including medical history and physical examinations. Diagnose and manage acute and chronic conditions. Order and interpret diagnostic tests. Prescribe medications within scope of practice. Develop and implement evidence-based treatment plans. Perform clinical procedures, including immunizations, wound care, and minor procedures. Provide preventive care and screenings. Educate patients on health conditions, treatment plans, and preventive care. Promote wellness, disease prevention, and healthy lifestyle changes. Maintain accurate documentation in the electronic health record. Collaborate with physicians, providers, and interdisciplinary care teams.
