Role Snapshot Transform bedside care with AI. As Circadia’s on-site NP, you’ll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions—reducing preventable readmissions. You’ll blend direct patient care with tech-to-bedside integration through hands-on training, live demos, and shadowed assessments. \n What You’ll Do Direct patient care: Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers. Tech → bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows, and run live demos during real patient encounters. Data into action: Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline. Close the loop: Document outcomes, refine protocols, and lead brief huddles to sustain adoption and measurable impact. High-volume rounding: Efficiently complete visits across designated SNFs (typ. 25+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts. Quality & product feedback: Surface field insights to improve workflows, summaries, and flagging logic; participate in root-cause reviews. Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams. Regional flexibility: Cover nearby sites (30–40-mile radius) as needed; mileage and lodging covered per policy.
