Description Opening Date: 12/01/2025 Closing Date: 12/15/2025 Job Title: Nurse Practitioner Salary/Wage: Negotiable Location: Siletz, Oregon Classification: Full-Time, Exempt Job Posting Number: 3745365 INTRODUCTION: The Nurse Practitioner serves as a medical provider at the Siletz Community Health Clinic. Providers at Siletz Community Health clinic use the medical home approach to deliver comprehensive care to both tribal members and community members. DUTIES: Evaluates total health care needs of patients, and with the patient, develops plans to meet those needs. Performs complete physical examinations. Secures a health and developmental history from the patient, and systematically and accurately records findings, and critically evaluates same. Develops care plan with patient that outlines care and treatment provided. Accurately and timely documents results of patient encounter. Ensures patient leaves encounter with understanding of care plan and instructions on when to follow-up. Performs diagnostic and therapeutic tasks, including injections, immunizations, and IV's. Delegates tasks as appropriate to care team RN or Medical Assistant. Instructs and counsels patients on diet, disease, and therapy. Provides full range of emergency services or crises intervention. Provides newborn and well childcare. Develops and coordinates programs for community health, preventive education, and employee health. Performs or requests special screening, developmental, radiology and laboratory tests, interprets, and evaluates results. Works with the physician in the management of selected complex medical problems. Exercises judgment in integrating and interpreting findings on the basis of general medical knowledge, and consultation with the physician, if needed. As necessary, may serve in other areas of incumbent's discipline. Requirements REQUIREMENTS: Graduate degree in nursing and license to practice medicine in State of Oregon. Family Practice nurse Practitioner is preferred. Knowledge of: Community health nursing principles, practices, and processes. Ability to: Ability to achieve and maintain appropriate Board certification and licensure with the State of Oregon. Develop and maintain positive working relations with the Clinic staff, tribal members, as well as outside health professionals. Possess, or be able to obtain, licensure and registration for the prescription and dispensing of narcotics and controlled drugs. Work with a wide variety of people, ability to communicate effectively, and relate to tribal and community members, staff, and federal, state, and local health agencies. Other: Possess, or be able to obtain, ACLS Certification if requested. Must be eligible for malpractice insurance. Must possess a valid Oregon State driver's license, insurance and be eligible to drive a GSA vehicle.
