Job Summary: The Nurse Practitioner (Primary Care) is responsible for providing comprehensive patient care services, including health promotion, disease prevention, diagnosis, and management of acute and chronic health conditions. This role works collaboratively with physicians and the healthcare team to deliver high-quality, patient-centered care consistent with CLS Health’s standards of excellence. Responsibilities: Perform comprehensive health assessments, including history, physical examination, and appropriate diagnostic testing. Develop, implement, and evaluate individualized treatment plans for patients. Provide care for acute and chronic medical conditions, ensuring continuity of care. Order, interpret, and evaluate diagnostic tests and laboratory results. Prescribe medications and treatments in accordance with Texas state regulations. Counsel patients and families on wellness, disease prevention, and management strategies. Document all patient encounters in the electronic medical record (EMR) accurately and timely. Collaborate with physicians, specialists, and other healthcare providers to coordinate patient care.
