Position Summary Holon Health is seeking an experienced and compassionate Nurse Practitioner licensed in Colorado to support individuals with SUD through collaborative, integrated care. This role combines telehealth-based care with periodic in-person presence across our Colorado locations (Denver, Fort Collins, Colorado Springs, and Basalt) to support our hybrid Care Navigator, Case Manager, and Peer Recovery Specialist teams. Travel frequency will vary based on client and team needs—we're looking for someone who values flexibility and is energized by both virtual and face-to-face connection. This position offers remote flexibility and can be structured as either full-time (40 hours/week) or part-time (20 hours/week) depending on your availability and interest. You’ll provide telehealth-based assessments, medication management, and care coordination to support clients on their recovery journey—and be part of a forward-thinking team breaking down barriers to behavioral healthcare. Essential Duties and Responsibilities Conduct health evaluations to assess substance use, co-occurring conditions, and overall functioning Diagnose health conditions and create individualized treatment plans Prescribe and manage medications, monitor for side effects, and adjust treatment as needed Collaborate with the Holon care team and external providers to deliver coordinated, whole-person care Serve as a clinical consultant on complex or high-need cases Educate clients and families on treatment plans, medications, and recovery support strategies Maintain accurate and timely documentation in compliance with ethical, legal, and medical standards Participate in team meetings, professional development, and continuous training Requirements Who You Are A licensed and experienced Nurse Practitioner Deeply compassionate toward individuals living with SUD and mental health challenges Experienced in addiction medicine or psychiatry, with a commitment to person-centered care A strong collaborator who thrives in integrated and team-based care models Comfortable with a hybrid model that includes periodic travel to multiple Colorado locations Adaptable to evolving schedule needs as our care model grows Organized, flexible, and capable of navigating complex systems Clear and confident in virtual communication with both clients and team members Education & Experience Active and unrestricted NP license in Colorado (multiple and additional state licenses are a plus) Active DEA license (and CDS where applicable) Several years of experience in behavioral health, addiction medicine, or psychiatry Board certification in specialty preferred Strong documentation skills and proficiency in EHR platforms The Holon Way We’re building a workplace that works for real life—because we know that when our people are well, they can do their best work.
