QUALIFICATIONS/SKILLS:

  • Must be registered in good standing with the College of Nurses of Ontario as a Nurse Practitioner
  • Masters in Nursing / Nurse practitioner from a recognized university
  • Minimum of 2 years of recent experience in primary care and/or community care
  • Certification to prescribe controlled substances.
  • BLS certification is required
  • Experience using Electronic Health Records Systems (EHRs). Experience with Telus Practice Solutions (PSS) is an asset.
  • Demonstrated strong communication and interpersonal skills
  • Demonstrated ability to work with a high degree of personal autonomy and as a member of an inter-professional team.
  • Possesses highly developed analytical, problem solving and critical thinking skills
  • Excellent oral, written, and presentation skills, with demonstrated competency in information and communication technology
Responsibilities
  • Practice within the full scope of the Nurse Practitioner role conducting primary care assessments, developing diagnoses and therapeutic plans of care.
  • Utilize skilled communication with patients, families, healthcare providers, and partners, to support coordinated, compassionate care, including Advance Care Planning and goals of care conversations.
  • Engage patients in therapeutic interventions that facilitate wellness and health promotion.
  • Collaborate with patients and inter-professional team members to develop, implement, and monitor coordinated care plans.
  • Prescribe medications, including controlled substances as indicated.
  • Demonstrate sound clinical reasoning using current evidence based practice from a primary care perspective.
  • Participate in quality assurance and program development initiatives
  • Assist with the development and implementation of medical directives.
  • Ensure that patient care standards are maintained according to College of Nurses standards, the Regulated Health Professions Act, and organizational policies.
  • Support education and development of inter-professional team members and learners
