QUALIFICATIONS

  • Valid Registered Nurse (RN) license and Family Nurse Practitioner (FNP) certification in the state of California.
  • Bilingual in Tagalog/English preferred
  • Master’s degree in nursing (MSN) from an accredited program with a focus on family practice.
  • Current DEA and BLS certificates are required.
  • Minimum of 1 year of experience as a Family Nurse Practitioner in a primary care or family practice setting.
  • Strong knowledge and proficiency in primary care, including health assessment, diagnosis, treatment, and patient education.
  • Must be COMFORTABLE with all age groups.

Responsibilities

JOB PURPOSE

As a Nurse Practitioner at a Federally Qualified Health Center (FQHC), you will be an integral part of the healthcare team, working under the guidance of the Medical Director. Your role involves providing comprehensive direct patient care services to individuals of diverse backgrounds and needs. This includes conducting thorough assessments, diagnosing illnesses, developing treatment plans, and prescribing medications when necessary. Additionally, you will play a crucial role in preventive care, health promotion, and patient education. Your dedication to compassionate, evidence-based practice will significantly fulfill Opsam Health’s mission of providing accessible, high-quality healthcare to underserved populations.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Perform comprehensive health screenings of patients of all ages.
  • Provides health care services to patients with acute and chronic medical conditions.
  • Order and review diagnostic and laboratory tests and refer patients to specialists as appropriate.
  • Complete progress notes in the electronic health record system in a timely manner.
  • Follow Opsam Health policies.
  • Keep abreast of current evidence-based medical knowledge.
  • Maintains minimum productivity requirements as set by the organization.
  • Maintains satisfactory peer and performance reviews.
  • Participate in quality improvement projects.
  • Performs other duties as assigned.

Salary: $70.00 - $90.00
