NURSE PRACTITIONER – LONG TERM CARE

The Nurse Practitioner is an integral part of the team, providing primary health care to the full scope of practice to our residents. The NP collaborates with other health professionals to identify and assess trends and patterns that have implications for our residents and families. As a member of the primary care team, you are accountable to the Administrator and Medical Director for meeting the Long Term Care Homes policies, procedures and protocols for medical services.
QUALIFICATIONS
- College of Nurses of Ontario registration in the Extended Class (NP-Primary Health Care or NP-Adult)
- Baccalaureate in Nursing (BScN), Masters Degree in Nursing (MScN or MN) preferred
- Knowledge of Nurse Practitioners’ Standards of Practice as set out by the College of Nurses of Ontario.
- Membership with Nurse Practitioners’ Association of Ontario (NPAO)
- Member of RNAO with professional liability insurance.
- National certification in Gerontology an asset
- Post graduate certificate and experience in palliative care an asset
- Experience or a combination of education/training in Geriatrics an asset
- Knowledge of RNAO Best Practice guidelines and MOH Regulations and Act
- Knowledge and understanding of the role of the Nurse Practitioner within an interdisciplinary team, how to optimize collaborative relationships
- Knowledge and proficiency in current, evidence-based methods and practices or primary care delivery with an emphasis on health promotion and disease prevention
- Demonstrated competency in advanced assessments, diagnosis and prescribing pharmaceuticals and non- pharmacological interventions within the legislated scope of practice
- Clear understanding of Resident-Centered Care

Responsibilities:
- 70% of the NP’s time must be engaged in direct care to residents, 30% to be engaged in research, education and leadership
- Provide primary care within the scope of practice of the Nurse Practitioner as outlined in the College of Nurses of Ontario Standards of Practice for Registered Nurses in Extended Class. The provision of primary care includes assessments, diagnosis, counselling, screening, referral, education, treatment and follow-up
- Collaborate effectively with physicians to facilitate appropriate provision of primary care outside of scope of practice
- Liaise with physicians and community agencies as required
- Maintain complete, accurate and timely records of patient visits using the electronic medical record (EMR)
- Communicates finding and diagnoses, discusses health outcomes to resident, families and the care team
- Provide individual and group sessions to patients in the understanding and management of particular health issues
• Prepare statistical reports for program review when necessary• Work effectively within an interdisciplinary team to improve patient access to primary care and to help improve the team’s ability to provide a broader range of services (e.g. wound care management)
- Evaluate and analyze the patient’s response to therapeutic interventions
- Participates in internal (e.g., Professional Advisory Committee) and external committees
- Provides leadership and involves the inter-disciplinary team in quality improvement initiatives
- Provides leadership in developing and implementing strategies to optimize the integration of illness and injury prevention, health promotion, health maintenance, rehabilitation and restorative care activities
- Provides formal and informal teaching and coaching in the management of clinical care to interdisciplinary team members, serving as a resource person, educator and role model
