NURSE PRACTITIONER (REGULATED HEALTH PROFESSIONAL, LEVEL 6) – HART HUB – FULL-TIME, TEMPORARY (2 YEARS) – BARRIE, ON

The Canadian Mental Health Association, Simcoe County Branch seeks a HART Hub NP, who, in accordance with the agency’s policies, standards, and guidelines, provides primary health care to clients accessing services through the Homelessness, Addictions, Recovery Treatment (HART) Hub in Barrie. This position will work with the CMHA NP Clinic and broader HART Hub program to integrate outreach-informed primary care with other health and social services delivered by CMHA and community partners.

REQUIRED QUALIFICATIONS:

  • Bachelor of Science in Nursing, as well as being a Registered Nurse Practitioner (NP) in good standing with the College of Nurses of Ontario.
  • Minimum three (3) years experience in mental health and/or addictions (community-based setting preferred).
  • Outreach experience preferred.
  • Must have access to a vehicle, a valid Ontario driver’s license, and adequate automobile insurance.
  • Fluency in French and/or other languages would be an asset.
  • Legally entitled to work in Canada

Responsibilities
  • Work with the CMHA NP Clinic to coordinate outreach-informed primary care to clients of the Barrie-based HART Hub and potential future HART Hubs. This could include both clinic-based care, in home and/or outreach as needed.
  • Manage 1.0 FTE RPN supporting the Barrie-based HART Hub.
  • Support obtaining and maintaining appropriate physician consultation in compliance with College of Nurses of Ontario standards.
  • Perform all aspects of NP nursing care, in accordance with standards of nursing practice established by the College of Nurses of Ontario to identified clients of the HART Hub.
  • Develop and maintain a process to assess and intake HART Hub clients requiring primary care.
  • Liaise with other primary care providers i.e., for HART Hub clients rostered to other providers.
  • Assist in the development and maintenance of effective referrals and connections with hospital and community service providers.
  • Refer patients to specialists, hospitals, community agencies, and/ or in-house staff as needed.
  • Work in collaboration with other CMHA SCB staff and programs to coordinate and advocate for additional services/resources to meet patient needs such as housing, financial, vocational, educational, social, recreational, medical, legal, ADL, etc.,.
  • Ensure clinical documentation is completed as required and that confidentiality is maintained at all times.
  • Write reports, support letters, and chart summaries for transfer and/or legal purposes, as well as other related patient and workload measurement documents, as required.
  • Ensure program-level adherence to all relevant policies and procedures.
  • Recommend and implement health, safety, and security procedures for staff and premises.
