NURSE PRACTITIONER, GAIN, 12 MONTHS, TO $110/HOUR

Status: Full-Time for 12 month Locum Contract with good potential for 24 months
Hours of Work/Schedule: 35 hours per week; 8:30 to 4:30 or 9 to 5 on Monday to Friday.
Remuneration: This is a Locum that will pay $90 to $110 per hour.
Position Reports To: Senior Manager, Integrated Care
Primary Work Location: Oshawa, free parking, and some Home Visits with a co-worker and mileage reimbursed

THE LOCUM QUALIFICATIONS

· experience working collaboratively with interdisciplinary teams
· experience working with patients with cognitive impairment and dementia related symptoms
· experience in a GAIN team, Regional Geriatric Program, Long Term Care preferred
· Assets include professional development in areas such as U-First, PIECES, Gentle Persuasive Approach, Mental Health First Aid, Dementia Foundations
· Able to drive with access to a vehicle to conduct home visits, as needed (mileage is reimbursed)
· Computer proficiency is required and experience with Telus PS Suites is an Asset
