Role: Nurse Practitioner (Part time 0.8FTE)
Reports to: Manager, Clinic Operations
Description: The Nurse Practitioner will function in collaboration with members of the Guelph Family Health Team, Physicians, patients, families, and external partners. The NP successfully functions in an expanded role demonstrating expert skill to formulate clinical decisions and appropriately manage acute/chronic illness and promote wellness. The NP utilizes a comprehensive theoretical knowledge base and an advanced level of clinical competence in caring for patients, involving collecting and interpreting data, determining a medical diagnosis, and treating complex responses of individuals and families to actual or potential problems. The NP performs quality improvement and administrative activities and participates in policy and procedure development.
Other key components of the role include performing research activities; delivering, coordinating, and participating in education and professional development activities/programs; performing activities to further develop and strengthen the profession; acting as a resource and serving as a consultant to individuals and group within the professional community.
