The purpose of this position is to maintain, improve, restore and optimize the health status of clients within an MHA transitional setting through direct client care for individuals and families. The Nurse Practitioner functions autonomously as an MRP within a collaborative practice setting while providing advanced holistic care of acute and chronic illness to clients across the lifespan. The Nurse Practitioner manages a patient caseload as an integral member of the Transitions Health Care Team. The team is composed of a variety of health care providers and team composition may vary dependent on identified needs. The Practitioner works collaboratively with all health care providers. Direct client services provided by the Nurse Practitioner include consultation with the treatment team, other clinicians and physicians, client physical and mental health assessment, diagnosis and treatment, assisting with the admission and discharge planning, health promotion, MHA campus clinics, and chronic disease prevention and management. This position is responsible for patient care on the MHA campus including Structured Programming, Addiction & Mental Health Transition Programs, Talbot House, Lacey House, Brackley Stables Transition o-=Home and Small Options Home. This position reports directly to the Manager of the MHA Wellness and Transition Center .
MINIMUM QUALIFICATIONS:
- Graduation from an approved and recognized nurse practitioner program.
- Eligible for licensure as a Registered Nurse with the endorsement as a Nurse Practitioner with the Association of Registered Nurses of PEI.
- Maintain CPR certification.
- Extensive nursing experience in a combination of hospital and community settings.
- Critical thinking proficiency to assess, analyze, diagnose, select treatment, evaluate, and refer patients with common acute or chronic health conditions, or for health promotion / illness prevention.
- Knowledge of practice setting programs and the community it serves, as well as current grasp of evidence-based approaches and epidemiology issues pertinent to the catchment area and available resources.
- Demonstrated ability to work effectively in an interdisciplinary environment with excellent collaborative, team building and consultative skills.
- Excellent communication (verbal and written), facilitation and interpersonal skills in dealing with clients, families, community groups, professional colleagues, and referral partners.
- Thorough knowledge and proficiency in medical and nursing physical assessment and treatment methods.
- Strong analytical, problem-solving and decision-making abilities.
- Must be available to work flexible hours and shifts such as evenings, nights and weekends as required.
- The successful applicant must meet minimal productivity requirement; demonstrated ability to be self directed.
- Must have access to reliable transportation.
- The successful applicant must have a good previous work and attendance record.
OTHER QUALIFICATIONS:
- Previous experience in a mental health setting preferred.
- Additional relevant education and experience will be considered an asset.
- The successful applicant must provide a satisfactory Criminal Records Check prior to beginning employment.
Salary Range: $57.94 - $65.59 per hour
Bi-Weekly Hours: 75.0 hours bi-weekly / Days / Evenings / Nights / Weekends when required / Rotating shifts
