The purpose of this position is to support person’s with ischemic stroke and chronic neurological conditions to access care through the neurology clinic. The incumbent will practice with the mentorship and support of the staff neurologists, and increase clinic capacity to optimize access to care in the neurology and stroke clinics by providing timely assessments and interventions as required.
The Neurology/Stroke NP will support intake of complex patients with neurological disorders requiring comprehensive assessments and medical histories. The NP will gain a strong understanding of patients neurological presentation and previous medical history. Clinical practice will involve intake and follow up assessments of patients that are identified collaboratively with the Neurologists. Follow up assessments on established patients can include and not be limited to patients diagnosed with. Multiple Sclerosis, headaches, Parkinson’s disease and others. The NP will provide support for ongoing symptom management and assessments if new neurological symptoms as they arise. There will be close collaboration with the neurologists to determine medical interventions and treatment strategies as well as further investigations. This role will support the patient’s overall function, quality of life, and long-term outcomes of their chronic neurological conditions. In the stroke clinic, the NP will do initial intake assessments and review of test results with patients as it pertains to lowering vascular risk factors and reducing recurrence of ischemic stroke. Follow up appointments may be arranged for management of HTN, DM, dyslipidemia, etc. especially for unaffiliated patients. This may also include building competencies and performing appropriate neurological assessments and procedures to improve patient outcomes. This position will include an onboarding phase during which the nurse practitioner will undertake specialized training as well as guidance and mentorship by the neurologists to develop the necessary clinical skills and competencies, foster confidence, and autonomy required to function within the neurology team.
