JOB DESCRIPTION:

Are you a dedicated Nurse Practitioner seeking a flexible role where you can make a meaningful impact on patients’ lives? Focus Care Inc. is looking for experienced Physician Assistants to join our team in conducting in-home health assessments. This per diem position offers you the chance to work independently, with compensation for each assessment, plus mileage reimbursement.

As a Nurse Practitioner with Focus Care, you will:

• Conduct comprehensive health assessments virtually, including medical history reviews, physical exams, and screenings for chronic conditions.
• Provide education on medication management, disease processes, and preventive care.
• Identify and document diagnoses to support care management and active treatment.
• Collaborate with primary care providers to communicate findings and address potential gaps in patient care.
• Flexibly schedule your appointments throughout the week, with no on-call, weekends, or holidays required.

MINIMUM REQUIREMENTS:

• 3 days a week minimum availability
• Valid Nurse Practitioner license in the state of Oregon.
• Board Certification from ANCC or AANP
• Minimum of 1 year of clinical experience; experience with geriatric populations preferred.
• Strong communication skills and a commitment to delivering compassionate care.
