Description ESSENTIAL DUTIES AND RESPONSIBILITIES To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. · Conduct in-home or facility-based visits to hospice patients for initial assessments, face-to face visits, routine follow-up visits, and symptom management. · Collaborate with the Medical Director or Associate Medical Director and the IDT to develop, implement, and revise individualized care plans. · Provide expert pain and symptom management consistent with hospice philosophy and guidelines. · Serve as a clinical resource to hospice nurses and other staff. · Participate in IDT as needed. · Educate patients and families on disease progression, end-of-life expectations and hospice treatment options. · Document all patient care accurately and timely in the electronic medical record. · Participate in on-call coverage as needed. · Comply with company policies, procedures, and guidelines. Location: La Grange, Texas Shift: PRN- up 12 hours per week Benefits: 403b Retirement and mileage reimbursement! Requirements · Minimum 2 years of NP experience · Master’s or Doctoral degree in Nursing from an accredited program · Current and unrestricted Nurse Practitioner license in the state of Texas · Certification as a Nurse Practitioner (AGNP, FNP, or ANP preferred) · Current DEA Registration and prescriptive authority in good standing; · Valid Texas Driver’s License, automobile insurance, and reliable transportation are required
Responsibilities
The Nurse Practitioner will conduct in-home or facility-based visits to hospice patients for assessments and symptom management. They will collaborate with the medical team to develop and revise individualized care plans and educate patients and families on hospice treatment options.
