POSITION DESCRIPTION

Position: Nurse Practitioner specializing in Primary Care and Chronic Disease
Management.
Location: Lac Courte Oreilles Community Health Center
Hours: Monday to Friday, 8:00 AM to 4:30 PM (with the option of working four
10-hour shifts)
Salary Range: Negotiable

Description of Position:
We are seeking a dedicated and compassionate Nurse Practitioner (NP) specializing in Primary
Care and Chronic Disease Management. The ideal candidate will play a key role in providing
high-quality, patient-centered care to individuals with both acute and chronic health conditions.
The NP will work closely with a multidisciplinary team to deliver comprehensive primary care
services, focusing on managing long-term conditions such as diabetes, hypertension, asthma,
heart disease, and more.
Qualifications:
Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an
accredited program.
Board certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse
Practitioner (AGNP).
Valid Nurse Practitioner license in the state of WI.
Certification through a recognized body, such as the American Association of Nurse
Practitioners (AANP) or American Nurses Credentialing Center (ANCC).
Experience in primary care or chronic disease management preferred.

Strong clinical knowledge of chronic conditions such as diabetes, hypertension, asthma,
cardiovascular diseases, and respiratory disorders.
Strong clinical skills and the ability to perform physical exams and diagnose medical
conditions.
Excellent communication skills to interact effectively with patients, families, and
healthcare teams.
Ability to work independently as well as part of a multidisciplinary team.
Knowledge of current medical practices, treatments, and guidelines.
Current DEA and DPS Certificate of Registration required.
Medicaid and Medicare provider eligible required.
Proficiency in electronic health records (EHR) and other clinical documentation tools.
Valid WI Driver’s License
Valid vehicle insurance
Be able to pass a pre-employment drug test and random drug screens
Be able to pass a background check
Ability to maintain confidentiality and observe all requirements of the Federal Privacy
Act and HIPAA regulations.

Responsibilities

Conduct comprehensive assessments, physical exams, and medical histories for patients
of all ages.
Diagnose and treat acute and chronic conditions, with a focus on chronic disease
management (e.g., diabetes, hypertension, cardiovascular diseases, COPD, etc.).
Order and interpret diagnostic tests (e.g., lab work, imaging).
Prescribe medications, treatments, and therapies as needed.
Provide patient education on managing chronic conditions and promoting overall health
and wellness.
Provide routine check-ups, screenings, and vaccinations.
Educate patients on self-management strategies, lifestyle modifications, medication
adherence for chronic disease management, and preventive measures to maintain health
and prevent disease.
Work in collaboration with physicians, other NPs, nurses, and healthcare professionals to
create and implement patient care plans.
Refer patients to specialists when necessary and follow up on their treatment plans.
Maintain accurate and up-to-date patient records in compliance with healthcare
regulations.
Document all interactions, assessments, and care provided in electronic health records
