JOB OVERVIEW

We are seeking a dedicated and compassionate Nurse Practitioner to join our healthcare team. The ideal candidate will possess a strong background in patient care and demonstrate proficiency in various medical settings, including pediatrics, acute care, and senior care. This role requires a commitment to providing high-quality healthcare services, managing patient cases effectively, and collaborating with other healthcare professionals to ensure optimal patient outcomes.

QUALIFICATIONS

• Valid Nurse Practitioner license with relevant certifications
• Experience with children and pediatrics is preferred
• Background in nursing home or senior care settings is highly desirable
• Familiarity with acute care practices and pain management techniques
• Strong knowledge of medical terminology and medication administration protocols
• Previous experience in case management is an asset
• Proficiency in using electronic health record systems such as Epic or Cerner
• Excellent communication skills with the ability to work collaboratively within a healthcare team
• Compassionate approach to patient care with a focus on improving health outcomes

  Pay: $140,000.00-$160,000.00 per year

• Conduct comprehensive patient assessments and develop individualized care plans
• Administer medications and treatments as prescribed, ensuring adherence to medical protocols
• Collaborate with physicians and other healthcare providers to coordinate patient care
• Provide education and support to patients and their families regarding health management and disease prevention
• Document patient interactions accurately in electronic health records (Epic, Cerner)
• Monitor patients’ progress and adjust treatment plans as necessary
• Perform diagnostic tests, including phlebotomy and laboratory procedures
• Manage cases for patients in various settings such as nursing homes, assisted living facilities, and home care environments
• Provide specialized care for populations including children, seniors, and individuals with dementia or chronic pain conditions
