Description Primary Care Nurse Practitioner COMPENSATION: Salary $145k + 15% of base We are a physician-led and mission-driven, primary care organization, is currently one of the most successful full- provider, transforming care of the neediest population. In order to achieve our vision and deliver our mission, we need the best primary care providers and clinical leaders that are seeking to fulfill purpose and personal opportunity and join the team. Transportation is provided to patients that need it at no charge. Role: • Compensation: $145k + 15% of base • Bonus: 15% of base • 18 patients a day • Hours: Monday – Friday work schedule; Clinic Hours 7:30a – 5:00p (last patient is scheduled at 4:15) • Work/Life Balance • Small Patient Panel – 450 patients’ max • Administrative time included in schedule • No nights or weekends • 100% outpatient • Dedicated Care and support team (have your own MA/Scheduler/ • Patient Population: Seniors with 3-5 chronic diseases • Value Based Care Benefits: • Paid Time Off • Comprehensive Benefit Package • Attractive Health, Dental, Vision and supplemental benefits plans for the provider in their family • 401K – 5.5% match Clinic offers: • Inhouse Consulting Specialist: Cardiology, Podiatry and more • Holistic Health Services: Acupuncture, Tai Chi, Yoga and Nutrition • Door to Door Transportation Requirements Requirements: • Board Certified Family Medicine or Geriatric Medicine • Primary care outpatient experience 2+ years

The Nurse Practitioner will provide primary care to seniors with chronic diseases, managing a small patient panel. The role includes administrative time and a focus on outpatient care with no nights or weekends required.
