JOB OVERVIEW

Community Physicians seeks an independent and self-motivated Nurse Practitioner or Physician Assistant. This is a rewarding opportunity to work closely with a team of experts in a high-volume outpatient setting focused on improving quality of life for patients with chronic pain.

QUALIFICATIONS:

  • Certified Nurse Practitioner (NP) or Physician Assistant (PA) in Illinois
  • Interventional pain management experience required
  • Experience with skilled nursing facilities
  • Strong clinical and communications skills
  • Able to work independently within a team-based model
Responsibilities
  • Conduct comprehensive patient assessments, including history, physical exam, and diagnostic evaluation.
  • Order and interpret imaging and diagnostic tests (such as x-rays, MRIs, and laboratory studies)
  • Perform minor, in-office procedures as appropriate.
  • Maintain accurate and timely documentation in the electronic medical records.
  • Provide patient education and counseling regarding treatment plans, medications, and wellness strategies as it relates to pain.
  • Contribute to quality improvement initiatives and team-based care as needed
