Real clinical ownership. ---------------------------------------- WHAT YOU’LL ACTUALLY DO: * Provide face-to-face care in patient homes and facilities * Make real-time clinical assessments that guide hospice eligibility and palliative decisions * Work directly with hospice physicians and attending providers to shape care plans * Educate families during some of the most important conversations of their lives * Support and elevate the interdisciplinary team with your clinical expertise * Document and communicate findings that directly impact patient comfort and care decisions ---------------------------------------- THIS ROLE IS FOR YOU IF YOU WANT: * Autonomy in the field—not clinic micromanagement * Time with patients instead of production pressure * A role where your clinical decisions actually matter immediately * A team that respects your expertise and input * Work that is meaningful, human, and purpose-driven Requirements * Current STATE RN and Advanced Practice Nurse License * Valid Driver’s License * Valid Auto Insurance * AHA CPR Certification * HomeCare HomeBase experience preferred
