This is a full-time outpatient opportunity within a value-based care model focused on adult and geriatric populations, emphasizing prevention, outcomes, and long-term patient relationships. POSITION OVERVIEW The Nurse Practitioner delivers comprehensive primary care in a collaborative, team-based environment. Providers manage their own patient panels while working alongside physicians, care managers, and clinical support staff to deliver coordinated, high-quality care.
