POSITION SUMMARY:
The Nurse Practitioner works collaboratively with the Complex Continuing Care and with the Cardiac Rehab inter-professional team in the medical management of ASCVD out-patients. The Nurse Practitioner will be a clinical resource and will provide quality care through advanced practice, education, research, leadership and consultation as outlined by the Facility policies and procedures and the Guidelines for Practice from the College of Nurses of Ontario.
SKILLS REQUIRED:
Ability to think problem solve, prioritize, demonstrate proficiency in computer communications, self-directed, independent work. Demonstrated excellence in communication, organization, & collaboration. The nurse will work in cardiac rehab, work with other RNs in the heart failure clinic, and some clinical research responsibilities.
Nursing Intakes, and out-take visits for Cardiac rehab
Other duties may be added as clinic needs require.
REQUIRED QUALIFICATIONS AND EXPERIENCE:
Master’s degree in Nursing, or a combination of education and experience and experience as sanctioned by the College of Nurses of Ontario, is required.

POSITION DUTIES & RESPONSIBILITIES:
Works within a defined scope of practice as an advanced practitioner within the auspices of approved medical directives and a practice agreement with physician partners
Devotes the majority of his/her time to direct patient care/issues, which includes monitoring of therapy modalities and interventions
Communicates and collaborates with the team, and the inter-professional care team with respect to assessment, diagnoses, treatment options, consultation possibilities, and discharge plans
Identifies and interprets diagnostic and screening tests in the assessment of common cardiac & other related health problems.
Initiates and performs a wide variety of investigations, diagnostic procedures and treatments from an approved list
Prescribes and initiates medications that are defined within the Nurse Practitioner’s scope of practice.
Utilizes critical thinking and diagnostic reasoning skills in managing patient’s/client’s needs for counseling, pharmacologic and or complementary therapies based on their presenting symptoms in both chronic and episodic illnesses
Acts as a clinical resource for staff if the cardiologist is not available.
Reviews all cases with attending cardiologist, either in person or virtually and develops a CarePlan in collaboration with the cardiologist.
Implements algorithmic care plans as developed in collaboration with the cardiologist.
Keeps abreast of current legislation, regulations, and professional issues withing College of Nurses of Ontario scope of practice.
Maintains continuing education in the areas of cardiology, diabetes, chronic disease, and area that pertain to the job description.
Maintaining CNO practice standards for NPs
