The Role: We’re seeking a Nurse Practitioner – Primary Care to provide comprehensive, patient-centered medical care across the lifespan. In collaboration with physicians and interdisciplinary care teams, you’ll assess, diagnose, treat, and manage a wide range of acute and chronic conditions while promoting health maintenance and disease prevention. Your clinical expertise and patient-focused approach will support our mission of delivering high-quality primary care in a compassionate and efficient environment. What You’ll Do: * Perform comprehensive physical examinations and health assessments. * Diagnose and manage acute and chronic illnesses. * Order, interpret, and evaluate diagnostic tests, including labs and imaging. * Develop and implement individualized treatment plans. * Prescribe medications and therapiesin accordance withstate regulations. * Provide preventive care, including immunizations and routine screenings. * Manage chronic conditions such as diabetes, hypertension, asthma, and heart disease. * Educate patients and families on disease management, lifestyle modifications, and preventive care. * Perform minor office procedures such as suturing, wound care, joint injections, and skin biopsies. * Coordinate care with specialists and other healthcare professionals. * Document patient encounters accurately andtimelyin the electronic health record (EHR). * Participate in quality improvement initiatives and population health management. * Other relevant duties as assigned by management. What We’re Looking For: * Master’s or Doctorate degree in Nursing (MSN or DNP). * Graduate of an accredited Nurse Practitioner program. * Current Registered Nurse (RN) license. * Current Nurse Practitioner license in the practicing state. * Nationalboard certification(AANP or ANCC) – Family NP or Adult-Gerontology NP. * DEA registration (if prescribing controlled substances). * BLS/ACLS may be required depending onsetting. * Strong clinical assessment and diagnostic skills. * Excellent communication and patient education abilities. * Knowledge of evidence-based practice guidelines. * Ability to work independently and collaboratively. * Proficiencywith electronic medical records (EHR/EMR). * Cultural competency anda patient-centered care focus. Work Environment: * Outpatient primary care clinic. * May include community health centers, private practices, or hospital-affiliated clinics. * Typical schedule: Monday–Friday, 8am–5pm.
