Support Culture of Excellence through active role with assigned neighborhood initiatives. Essential Job Functions: * Performs and documents pre-admission/admission, routine, and emergency Member evaluations on selected Members in collaboration with the in-house attending physicians. * Collaborates with Nurse Mangers and other Health Team members to coordinate patient care plans and to improve quality of Member care. * Participates in Patient Care Team Meetings as requested. * Ensures documentation for care planning process per New York State requirements. * Provides counsel, support and education to Members and their families in collaboration with the Interdisciplinary Team. * Conducts pre-employment physical exams and maintains surveillance of the health status of the facility's employees under direction of the Medical Director. * Evaluates employees prior to returning to work as requested by Human Resources under the supervision of the Medical Director. * Participates as a member of the In-house Psychiatric Team and the Skin Care Team. * May serve as a preceptor for approved clinician students. * Participates in Quality Assurance programs. * Performs other duties as required by Administration from time-to-time. Other Job Functions:   Expectations of the Position: * Dependable, report to work on-time for every schedule shift (ready to work at beginning - start of shift) and work full schedule shifts unless physically unable or pre-approved Paid Time Off (PTO) which includes personal and vacation. * Willing and able to work overtime and weekends as required by the position and/or approved by management. * Have reliable transportation to and from work. * Must be clean and neat in appearance and have a “friendly” service-oriented demeanor and supports Culture of Excellence initiatives by interactions with co-workers and/or Members. * Abide by work-related / job procedures and organization policies; (refer to the Employee Handbook). Qualifications of the Position: * Education:  Graduation from an approved school of nursing.  Master’s degree and ANA Certified Nurse Practitioner. * Experience:  Past experience as a Nurse Practitioner, preferably in geriatrics. * Skills:  Ability to meet the public and participate in professional programs. * Licenses / Certifications:  Current NYS Registration and License.
