Nurse Practitioner or Physician Assistant – Home-Based Primary Care Job Type: Full-time or Part-time Compensation: $150,000 – $250,000 per year (based on experience and productivity) Senior Doc is hiring a Nurse Practitioner (NP) or Physician Assistant (PA) to join our mobile primary care team. This role is perfect for advanced practice providers who value autonomy, flexibility, and meaningful relationships with their patients—outside of the traditional clinic environment. Our fee-for-service model allows providers to focus on what truly matters: delivering relationship-based, comprehensive care to seniors in need. Key Responsibilities Conduct in-home visits to provide primary care services across residential and community-based settings. Complete detailed health assessments, manage chronic and acute conditions, and develop personalized care plans. Collaborate with specialists, ancillary services, and caregivers to ensure coordinated, patient-centered care. Maintain accurate documentation using our secure EMR system. Work independently while supported by a responsive clinical and administrative leadership team. Qualifications Active and unrestricted NP or PA license. Board certification (AANP, ANCC, or NCCPA). Experience in geriatrics, internal medicine, family practice, or home-based care preferred. Valid driver’s license and reliable transportation. Strong clinical judgment, time management, and communication skills. Passion for serving older adults with empathy and excellence. Compensation & Benefits Fee-for-service model with total annual earning potential between $150,000 – $250,000+. Sign-on bridge provided to support providers during the initial ramp-up period.
