Description Summary of Responsibilities: The Nurse Practitioner/Physician Assistant provides routine care and management of the patient with acute and chronic complaints, in collaboration with the supervising physician and through implementation of standardized procedures. Essential Duties and Responsibilities: Provide general medical services and treatment to patients in the clinic under direction of physician. Perform physical examinations and preventive health measures within prescribed guidelines and instructions of physician. Make initial and continuing assessment of patient's health status followed by appropriate intervention independently or by collaboration with other physician specialists. Order, interpret, and evaluate diagnostic tests to identify and assess patient's clinical problems and health care needs. Record physical findings, and formulates plan and prognosis based on patient's condition. Discuss case with physician and other health professionals to prepare comprehensive patient care plan. Submit health care plan and goals of individual patients for periodic review and evaluation by physician. Prescribe or recommend medication or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures. Establish and maintains client's electronic medical records and other necessary records. Collaborate with the Contract Health Services coordinator to provide referral services to specialty clinics/services Perform other duties as required. Requirements Education and Experience: Graduation with a master's degree from an accredited advanced nurse practitioner program of Physician Assistant program. Board Certified Family Practice Nurse Practitioner or Physician Assistant Minimum of three years in practice preferred Knowledge, Skills and Abilities: Knowledge of the information and techniques needed to diagnose and treat human injuries, diseases, and deformities. This includes symptoms, treatment alternatives, drug properties and interactions/ and preventive health-care measures. The ability to communicate information and ideas in writing so others will understand. Identify complex problems and reviewing related information to develop and evaluate options and implement solutions. Proficient in Microsoft Office applications. Excellent patient care skills. Knowledge of professional care theories/ principles/ practices/ disease process and procedures. Knowledge of a variety of pharmacological agents used in patient treatment/ the desired effects/ side effects/ and complications of their use as well as the accurate administration of the pharmacologic agent including dosage calculations as required. Knowledge of the administrative requirements for proper documentation of patient’s condition including disease progress/ acknowledgement of teaching/ and follow up care. Knowledge of computer operations and proficiency in the use of basic word processing/ data entry and automated medical records. Annual TB skin test/ proof of previous positive PPD results or negative chest X-ray. Hepatitis A & B, Tdap, MMR and Varicella vaccinations, proof of previous vaccinations or proof of positive titer.
