QUALIFICATIONS:

  • Graduate degree in Nursing (Nurse Practitioner) or Physician Assistant Studies.
  • Valid Michigan driver’s license.
  • Current Michigan NP or PA license.
  • Current national certification (AANP/ANCC for NPs; NCCPA for PAs) in Adult, Geriatrics, or Adult Gerontology Primary Care.
  • Minimum 2 years’ experience in Neurology or Psychiatry.
  • One or more of the following:
  • Specialty designation in Neurology, Psychiatry, Geriatrics, Geriatric Psychiatry, Behavioral Neurology, or Geriatric Neurology.
  • At least 25% of patient panel in the last 5 years comprised of adults with cognitive impairment (including dementia).
  • At least 25% of patient panel in the last 5 years comprised of adults aged 65 or older.
Responsibilities

ABOUT THE ROLE:

We are seeking a compassionate and skilled Nurse Practitioner or Physician Assistant to deliver holistic, patient-centered medical care in home and community-based settings, such as adult foster care homes. You will collaborate with a multidisciplinary team-including nurses, social workers, behavioral health counselors, dieticians, and the medical director-to improve patient health outcomes, engagement, safety, and quality of life.

RESPONSIBILITIES:

  • Build positive rapport and engagement with patients and coworkers while ensuring excellent customer service.
  • Conduct comprehensive clinical assessments, including medical history, medication reconciliation, vital signs, and physical exams.
  • Maintain accurate and timely clinical documentation.
  • Review and interpret medical, surgical, and social history; perform appropriate screenings.
  • Diagnose conditions, develop and update individualized Plans of Care, and involve patients and support systems in decision-making.
