Job Description
Position Title: Nurse Practitioner
Status: 22.5 hours weekly Monday-Friday (future potential for up to full time), $62.66 hourly
Reports To: Executive Director
Position Summary
The Nurse Practitioner will function as a member of an interdisciplinary team providing direct nursing care to clients by assessing and promoting health and wellness to individuals of all ages, families, groups, communities, and taking action to enact our vision of better health for all.
The Nurse Practitioner will actively participate in the development and implementation of health promotion programs with individuals, families, and community groups.
This incumbent will make decisions and accept legal responsibilities, within the scope of responsibility for Registered Nurses in the Extended Class, for all aspects of clinical services required. The Nurse Practitioner is expected to uphold and maintain the professional and organizational core competencies necessary for practice in the primary care setting.
Duties and Responsibilities
Patient Focused
· Prescribe medication according to scope of practice
· Contribute to electronic medical records
· Collaborate with members of an interdisciplinary health team to plan, implement, co-ordinate and evaluate client care in consultation with patients and their families
· Assist in patient navigation of program streams within the Family Health Team (FHT)
· Refer patients to the appropriate providers or resources within the organization and externally
· Provide health education and participate in community needs assessment and program development
Team Focus
· Consult as necessary with FHT physicians
· Provide consultation and clinical support to peers as needed
· Assist in the orientation and preceptorship of new staff and students
· Communicate with community agencies as necessary
· Effectively utilize human and material resources
Scope of Practice and other expectations
· Function within the scope of practice as defined by the College of Nurses of Ontario
· Adhere to PrimaCare Community Family Health Team (PCCFHT) policies and procedures
· Participate in quality improvement and program evaluation projects
· Act in accordance with Ontario’s Personal Health Information Privacy Act to protect clients’ privacy and confidentiality
· Must possess a willingness to participate in regular professional development including training requested by the FHT
· Ability to travel between 3 sites, as required
Education, Skills and Experience
EDUCATION:
EXPERIENCE:
· 2 years of community health or primary care experience preferred
ADDITIONAL SKILLS/ABILITIES:
· Commitment to client-centred care
· Demonstrated ability to work in a collaborative practice model with multidisciplinary teams
· Demonstrated ability to use initiative and work autonomously utilizing critical thinking skills and advanced decision-making, problem recognition and problem-solving skills
· Experience with program development, quality improvement initiatives, and program evaluation
· Effective organizational, interpersonal and communication skills, and demonstrated negotiation and conflict resolutions skills
· Knowledge of Microsoft Office and Electronic Medical Records
· Recognition of the limitations of own competencies and scope of practice, and when to consult or refer to another healthcare provider
