Job Title: Nurse Practitioner (Primary Care – Functional Med/Holistic Care/Traditional)
Location: Mesa, AZ
Patients/Day: 10-12 Max

HOURS & SCHEDULE:

  • Mon & Wed: 8:00 AM – 2:00 PM
  • Tues & Thurs: 12:00 PM – 6:00 PM
  • Fri: 8:00 AM – 6:00 PM
  • 1 Saturday per month: 8:00 AM – 2:00 PM
    Work Environment: Outpatient Clinic
    Salary / Hourly Rate: $100,000 – $130,000 / year
    Bonus Offered: Annual Bonus
    Benefits Offered: UHC Medical ($1500 deductible, 80% employer covered, 50% family), 1 Week Sick Time, 1 Week PTO (increases annually)
    On Call Required: Minimal rotating after-hours call

We’re looking for someone who is:

  • DNP or FNP licensed (new grads welcome)
  • Ideally credentialed or willing to complete credentialing (may begin at lower pay during this time)

Job Summary:
As a Primary Care Nurse Practitioner, you’ll work with a diverse patient population—primarily Medicare, with some commercial and cash-pay. You’ll see a maximum of 10–12 patients per day, providing thoughtful, in-depth care. This is a great fit for a provider who wants to make a real difference, taking the time to get to know their patients and help them feel heard, seen, and healed. You’ll work closely with a collaborative team dedicated to long-term health outcomes and whole-person care.

Job Duties & Responsibilities:

  • Provide comprehensive primary care with a mix of traditional, functional, and holistic approaches
  • Evaluate and manage acute and chronic health conditions
  • Work to identify root causes and long-term health solutions for patients
  • Educate patients about their health and treatment plans
  • Coordinate care as needed and refer to specialists when appropriate
  • Maintain accurate, timely documentation in the EHR
  • Participate in minimal rotating after-hours call coverage
  • Build long-term relationships with patients and provide continuity of care

Prerequisites / License & Certification Requirements:

  • Active and unrestricted DNP or FNP license in Arizona
  • DEA license (negotiable for reimbursement)
  • Malpractice insurance (must be self-provided, negotiable)
  • Credentialed or willing to undergo credentialing
  • Open to W2 or 1099 employment structure
