Description We’re looking for a Nurse Practitioner who loves primary care and values building real relationships with patients. Our busy, well-established primary care practice in the Farmington Valley is growing, and we’re excited to add a compassionate, team-oriented APRN to our group. This is an outpatient position with a strong support team and an established patient base. You’ll have the opportunity to care for a diverse adult population, manage both acute and chronic conditions, and truly make a difference in your patients’ lives. Responsibilities

Responsibilities also include developing treatment plans, educating patients on wellness, and maintaining accurate EMR documentation.
