QUALIFICATIONS:

  • Current valid CNO registration as an RN (EC)
    -BScN required, - Master’s degree in Nursing from an accredited program
  • Relevant clinical and health assessment experience as a licensed Nurse
  • Current CPR certification
  • Certified Diabetes Educator (CDE) certification an asset
  • Certification as a Nurse Practitioner in the appropriate specialty area (e.g., Family, Adult or Gerontology)
  • Experience working with elderly patients with memory care needs or dementia is preferred
  • Strong clinical skills and knowledge of evidence-based practice guidelines
  • Excellent communication and interpersonal skills to effectively collaborate with patients, families, and healthcare teams
  • Ability to prioritize tasks, manage time efficiently, and work independently

Responsibilities
  • Provide primary and acute medical care to seniors in Retirement Homes working at full scope of practice in the Mississauga area.
  • Conduct comprehensive assessments, diagnose and treat acute and chronic illnesses
  • Conduct person-centered comprehensive health histories
  • Collaborate with interdisciplinary healthcare teams to develop and implement patient care plans
  • Collaborate with the resident nursing staff and their family, to develop a person-centered plan of care
  • Regular medical assessments with patients
  • Interpret diagnostic tests, such as specimen collection processing and medical imaging
  • Participate in family care conferences
  • Educate staff, patients and their families on disease prevention, management, and treatment options
  • Provide specialized care for patients with , dementia, and other complex medical conditions
  • Early identification of illness allowing timely intervention and reduce hospital transfers
  • Resident Support & Education - age specific health promotion, education, disease and injury prevention support
