Join our team!   We are looking for a passionate Nurse Practitioner NP to join our leading team of hospice care professionals and make a difference, by listening, caring, and serving.    POSITION SUMMARY: The Nurse Practitioner works in collaboration with a physician and other palliative care team members to deliver palliative care and related health care services to chronic and serious ill patients and families.    QUALIFICATIONS: * Graduate of an accredited School of Nursing and accredited Nurse Practitioner Program. * Current state RN and APN license and CPR certification. * Minimum 2 years’ experience as an APN clinical care setting preferred; hospice and home health preferred. * Certification in a specialist area preferred; (e.g., Hospice and Palliative Nursing (CHPN) or Geriatrics) * Excellent observation, verbal and written communication skill, problem solving skills, nursing skills per competency checklist. * Reliable transportation, valid driver’s license, and current auto liability insurance coverage.   This is not a comprehensive list of all job responsibilities; a full job description will be provided.
