Project HOME is seeking a Certified Nurse Practitioner to join our integrated healthcare team, providing high-quality, patient-centered care to individuals experiencing homelessness, our residents, neighbors, and staff.
At Project HOME, you’ll do more than practice medicine- you’ll be part of a mission to break the cycle of homelessness and poverty through healing, advocacy, and hope.

• Deliver primary care to children and adults in a supportive, team-based environment—both in-person and via telehealth.
• Collaborate with an inspiring care team that includes behavioral health consultants, nurse care managers, peer specialists, and community health workers.
• Lead care coordination efforts that address not only clinical needs but also the social determinants of health.
• See approximately 9 patients per session, across 8 half-day sessions weekly.
• Contribute to program development, mentor the next generation of providers, and participate in community-wide quality improvement initiatives.
• Attend monthly staff meetings, present patient cases biannually, and uphold the mission and values of Project HOME in everything you do.
