St. Joseph’s Health Centre, Unity Health Toronto is seeking a Nurse Practitioner to join the Inter-professional team for the Reactivation Care Unit located at Humber Church site (200 Church St.; near Jane Street south of the 401). The Reactivation Care Unit cares for adult and geriatric patients who require therapy, reactivation and transition planning in order to transfer to the community and/or other supportive environments.

POSITION SUMMARY:
The Nurse Practitioner (NP), Reactivation Unit, works with the inter-professional team and physician partners in the Seniors Health and Ambulatory Program to meet the unique needs of our patients. With a focus on medical management and optimizing function and discharge, the NP works to full scope of practice within the Reactivation Unit to support the medical and psychosocial needs of the patients.
The NP will primarily work at the Humber Church site but may be required to support the Reactivation Care Unit at the main hospital site at any given time, depending on operational needs. The NP is expected to support appropriate tasks and duties across both locations in alignment with operational needs, with the understanding that service delivery requirements may evolve over time.
The NP provides leadership and promotes excellence in nursing through the roles of practitioner, consultant, educator, researcher, and leader. By utilizing theoretical and clinical expertise in this specialty area, the NP delivers advanced practice nursing to patients and families with complex situations within the in-patient setting.
The NP contributes to the education and professional development of nursing/inter-professional staff, students and the professional community within these areas of specialization, and participates in academic and research projects. The NP integrates research into practice to ensure best practice standards are maintained and incorporated into clinical initiatives.

The Nurse Practitioner (NP) will utilize a comprehensive theoretical knowledge base and an advanced level of clinical competence in caring for inpatients on the Reactivation Unit within a philosophy of patient-centered care. This will involve health assessment and diagnosis of medical and cognitive problems, developing and implementing a plan of care, providing health counseling and emotional support in the areas of health promotion and the prevention of illness, injury and complications.
